The film opens with Steve, Natasha, and a S.H.I.E.L.D. counter-terrorism team (STRIKE) led by Agent Brock Rumlow on a mission to rescue hostages from a hijacked S.H.I.E.L.D. vessel. During the mission, Steve discovers Natasha has a separate, covert objective from Director Nick Fury to extract data from the ship, which raises his suspicions about S.H.I.E.L.D.'s transparency.
Back at the Triskelion, S.H.I.E.L.D.'s headquarters, Steve confronts Fury about the data. Fury, increasingly wary himself, reveals Project Insight: three new Helicarriers linked to spy satellites designed to neutralize threats before they emerge, based on a sophisticated algorithm developed by Arnim Zola. Fury's growing distrust leads him to ask Alexander Pierce, a senior S.H.I.E.L.D. official and World Security Council member, to delay Project Insight.
Before Fury can fully investigate, he is ambushed by assassins led by the enigmatic Winter Soldier. Fury barely escapes to Steve's apartment, where he warns Steve that S.H.I.E.L.D. has been compromised and hands him a flash drive containing the encrypted data, telling him to trust no one. Moments later, Fury is seemingly assassinated by the Winter Soldier.
Pierce questions Steve about Fury, but Steve, remembering Fury's warning, refuses to divulge information. Branded a fugitive, Steve escapes from the Triskelion, fighting his way through STRIKE agents, including Rumlow. He meets up with Natasha, and together they use the flash drive to find a hidden S.H.I.E.L.D. bunker beneath Camp Lehigh, the old military base where Steve trained.
Inside the bunker, they activate a supercomputer that houses the preserved consciousness of Arnim Zola. Zola reveals a shocking truth: after his capture in WWII, Hydra secretly infiltrated S.H.I.E.L.D., growing within its ranks for decades. They manipulated global events, including the creation of Project Insight, to identify and eliminate millions of individuals deemed "threats" to their totalitarian agenda. Zola explains that the Winter Soldier is Hydra's deadliest assassin, and that his algorithm would ensure a new world order. A S.H.I.E.L.D. missile then destroys the bunker, but Steve and Natasha narrowly escape.
On the run, Steve and Natasha enlist the help of Sam Wilson (Falcon), who aids them with his advanced "Falcon" wingpack. They capture Hydra operative Senator Jasper Sitwell, who confirms Zola's revelations about Project Insight. They then face the Winter Soldier again, and in a brutal fight, Steve manages to tear off the assassin's mask, revealing his true identity: Bucky Barnes, Steve's best friend, presumed dead in World War II, but brainwashed and cybernetically enhanced by Hydra. Bucky, however, shows no recognition of Steve.
With Fury's help (who faked his death with a specialized serum), the trio plans to expose Hydra. On the day of Project Insight's launch, Steve broadcasts Zola's confession to the entire Triskelion. Natasha, disguised as a World Security Council member, disarms Pierce, and Fury confronts him. Steve and Falcon infiltrate the Helicarriers, replacing their control chips to force them to target each other instead of innocent civilians.
Steve faces Bucky on the final Helicarrier, refusing to fight his old friend. He sacrifices his own safety to reach the chip, appealing to Bucky's fragmented memories. As the Helicarriers crash, Steve is plunged into the Potomac River but is rescued by Bucky, who then disappears.
In the aftermath, S.H.I.E.L.D. is dismantled, and Natasha Romanoff leaks all of S.H.I.E.L.D. and Hydra's classified files to the public, exposing the corruption. Fury goes underground to hunt remaining Hydra cells. Steve and Sam begin their search for Bucky, hoping to restore his memories and save him from Hydra's control.
A mid-credits scene shows Baron Wolfgang von Strucker examining Loki's scepter and two enhanced individuals, Wanda and Pietro Maximoff, hinting at future threats. A post-credits scene shows Bucky visiting the Captain America exhibit at the Smithsonian, beginning to piece together his past.
